Patient Portal Ezyaccess Instructions
Dear Family Health Center Patient Portal User:
Your health provider is using Electronic Medical Records through our web based program called myezyaccess. You can now view your test results, communicate with your nursing staff, request appointments, confirm and/or cancel your existing appointments, pay your bills, update personal information, request medication refills, view your office visits, request shot records and referrals. In addition, if you have a personal printer at home, all records are available for you to print. We can improve the quality of your communication with our office staff, maintain your patient privacy and protect your health information.
Step 1. How do I sign up? * Call the Family Health Center (580) 226-0543 and request portal access by giving your email address to the receptionist.
Step 2. Welcome to the Portal * Once your email is added into our system, you will receive an email with the subject line: Family Health Center Patient Portal Access . If you do not find this email in your Inbox, please check your Spam folder. * Open this email and click on the link https://familypracticeassoc.myezyaccess.com * You will be redirected to the ezyaccess webpage * Use your email address as your User ID and the temporary password listed in the email * You will prompted to change your password. Please use 8 to 12 characters * You will need to choose a security question from the drop down box and enter the answer
Step 3. Navigation * You may use the tabs along the right side of your screen * Any updated information or documents will be in the message tab
*NOTES* 1. We recommend setting up your access on a desktop/laptop as it is much easier to navigate through the set up process than using a smart phone. 2. You must have Adobe Acrobat Reader in order to view documents. 3. Results of office visits, lab tests, radiology tests, etc. are only available for viewing 24 hours AFTER the provider has reviewed and signed them. They will automatically upload to your documents. 4. Our staff will no longer call to remind patients of appointments, RX pickups, etc. So please log into the portal to see messages from our staff, your next appointment date and documents.
